I understand that I am a volunteer for the Spike Frog Volleyball and am expected to conduct myself in a professional manner relative to the activity I am participating in. I understand the use or possession of alcohol or narcotic drugs prior to or during the activity I am volunteering for is prohibited. I further understand I will not receive any type of compensation for my volunteer services and that I am not covered under Workers' Compensation should I suffer any type of illness or injury. I also agree to complete a Background Check, provided by Spike Frog Volleyball.

I understand that information contained in my application will be verified by Spike Frog Volleyball. I understand that misrepresentations or omissions may be cause for my immediate rejection or termination as a volunteer.
