Volunteer Background Check for Spike Frog Volleyball Leagues
Spike Frog Volleyball requires all volunteers over the age of 21 to submit to a criminal background check every 2 years. As a service to our athletes, we offer this free of charge to our volunteer coaches who will be working with Spike Frog athletes.

We have partnered with IntelliCorp Records, Inc to handle our criminal background checks.
Once you click on the link below, you will be redirected to a standard disclosure and authorization screen. PLEASE READ CAREFULLY. The Fair Credit Reporting Act (FCRA) mandates that this exactly worded screen be presented to every applicant before a search may be executed.
The language on this screen references a "consumer report" for employment purposes. Spike Frog Volleyball has no interest in obtaining any credit information on their volunteers. As such, a Spike Frog Report will ONLY contain
· Social Security Number Verification/Address History
· Government Sanctions (OFAC, DDTC, AECA, BIS, GSA)
· Nationwide Sex Offender Registry
· Multi-State Criminal Search, Single County Searches
You may wonder what a "Government Sanction" is. Intellicorp describes these searches thusly.
The Government Sanctions search helps ensures that organizations and individuals have not entered into transactions with a person involved in fraud, terrorism, international narcotics trafficking, activities related to the proliferation of weapons of mass destruction and other sanctionable actions. Results include data from government agencies including (but not limited to) the Office of Foreign Assets Control (OFAC) , Government Services Agency (GSA), Directorate of Defense Trade Controls (DDTC), Bureau of Industry and Security (BIS), and the Bureau of International Security and Non-proliferation.

You should select "Texas" as your state/Province of Work/Volunteer.
Click Next to submit your information. PLEASE REVIEW your information for accuracy and fix any errors before clicking on "Complete" to submit your application.
On the final screen you will see a "Thank you for using the Spike Frog Volleyball portal." message. You have a right to see your report once the check is completed. If you click on the link marked "Request a free copy of your report" you will be able to enter in your information again (sorry, apparently Intellicorp does not have a way to keep that information cached for the next screen) and you will receive an EXACT copy of the report that Spike Frog Volleyball receives. It may take up to 2 weeks, as indicated.
Should there be any issues with your background check, Intellicorp will investigate by contacting the exact agencies/courthouses to verify the information. As such, there may be a delay. If they verify the information and find issues with your volunteer application, Spike Frog Volleyball will be notified.
We will not allow any individual to work with our athletes who do not have a cleared background check. We will simply notify you that we will not be using you as a volunteer for our league.
